1. Application form duly attested by Member of Parliament, MLA, Municipal Councilors or Gazetted Officers, Member of SC/ST/OBC welfare Board certifying the caste of applicant. Copy of Ration Card or other Proof of Residence.
2. Birth Certificate or School Certificate showing date of birth or if applicant is illiterate, an affidavit declaring age.
3. No Affidavit is required to be furnished by the applicants who append copy of SC Certificate of their father side issued from Delhi state. However, in case of applicants who applied on the basis of SC Certificates issued by other states the affidavit is required
4. In case applicant is a married woman, proof of residential address before marriage. Copy of SC Certificate of father / brother/ sister, if any.
5. Copy of SC Certificate of father / brother/ sister, if any.
6. Where no SC/ST Certificate has been issued to any member of applicant’s family, two witnesses who are Government servants are required who should give in writing that they know the person and his caste, alongwith the attested copies of their identity Cards. 
7. Documents at Sr. No. 2 to 7 to be attested by a Gazetted Officer . 
8. Two Passport Size photographs, one attested and one plain

In case where the applicant is a permanent resident of Delhi & has appended a copy of SC/ST certificate issued to any member of his family, no local inquiry is conducted. In such case, verification is done from record available in office of SDM or CCS branch in office of Divisional Commissioner. Where the record is not available in office of SDM or CCS branch, a local inquiry is conducted.

In case, no member of the applicant’s family has been issued a certificate earlier, a local enquiry is conducted.

In case of a married woman, the local enquiry is conducted at the place of residence prior to marriage as well as the place of residence after marriage.

Such certificate cannot be issued to a person if he is already in possession of such certificate issued by the Competent Authority.
