Q. Where do I have to go and during which hours?

Q. Which papers/documents/fees, do I take with me?
- Application on a plain paper.
- Copy of Ration Card.
- Affidavit regarding residence, occupation, property owned and income. Click here for proforma of Affidavit.

Q. What will be the criteria used while deciding my case?
A local enquiry is conducted through bailiff / other staff to verify the particulars mentioned in the application and affidavit. The certificate is issued after receipt of the report. |
