Application on plain paper. |

Affidavit to the following effects:- Details of deceased person i.e. name, age, address. Details of surviving members of deceased, their ages and relationship with the deceased. Purpose for which the certificate is required. For proforma of affidavit, Click Here |

Death Certificate. |

Photocopy of ration card/any other proof of residence in Delhi. |

Copy of letter/reference of the authority before whom the surviving members certificate is required (if any). | |

| Q. What will be the criteria used while deciding my case? |
| |
| After receiving the application, verification of the facts is carried out through the Tehsildar. On the basis of the report of Tehsildar & documents submitted by the applicant, the certificate is issued to the applicant. |
