INTRODUCTION

 

School lays a country’s foundation for the future and has a major effect on host of issues, including health. Science of psychology tells that what is learnt at this “impressionable stage of life” in terms of knowledge, attitudes and behaviors has a lasting impact on the entire lifespan of the individual. Health is maintained and diseases most effectively controlled by knowledge – knowledge of prevention of disease and early recognition of signs of sickness. Ensuring that children have access to knowledge ensures that they are able to change not only their own attitude and behavior, but that of their families and communities as well. This is the primary reason for health issues occupying the center stage in the school curriculum.

Schools play a vital role in the overall development of a child into a competent productive adult who contributes usefully to society. Health is an important aspect of development of children and education is an important determinant of health. Almost all children attend school at some time during their lives and spend 6-7 hours of their time everyday in that learning environment. Apart from this, the school curriculum can have substantial influence on health promoting behaviors. Schools are often the strongest social and educational institutions available for execution of interventional programs as they have the required structure and governance. Schools have profound influence on thinking patterns and behaviors of children, their families, and the community in general.

 

Providing easy access to health to children in schools, nutrition and hygiene education and services is a simple and cost effective tool which can go a long way in the prevention and control of communicable and non communicable diseases Population stabilization, gender and demographic balance are other issues that the country has been grappling with since independence. The tender formative years at school could well be the launching pad for addressing these concerns in the years to come. India is developing country; importance is not given to health as required because of poverty and socio cultural milieu. A substantial number of school children from pediatric age to adolescent suffer from various diseases, which can be prevented if diagnosed and treated early and preventive measures taken in time.

 

Keeping this in view the Directorate of Health Services, Govt. of NCT Delhi, started School health scheme in March 1979 with six School health clinics initially which were later increased to provide comprehensive health care services to the school going children. The School Health clinics were established in the school premises itself for easy accessibility to children.  These were planned to cover the children studying in Government and Government Aided Secondary schools.

Implementation of the School Health Scheme

Implementation of the SHS is through the Medical Officers who head the school health team in School Health Clinic and in Sub Clinics. The school health team is responsible for the conduct of health examination and undertakes the following activities to change Attitudes, Beliefs and Practices of School Children by providing them adequate knowledge about healthy practices and developing healthy habits amongst them. The  strategies used are.

·        Child to Child dissemination of information

·        Child to Family/Parents dissemination of
information

·        Child to Community dissemination of information

FUNCTIONING OF SCHOOL HEALTH SCHEME

The scheme is headed by Joint Director (In-charge) to carry out the day to day   functioning of school health clinics /special referral Centre, which include planning, implementation, administration and monitoring. 

 School Health Clinics -School health clinics are situated within the school premises. Each school health clinic caters to about 10 thousand children i.e a cluster of 8 to 10 schools  located around
radius of 3-4 Km.

Each District has one Special referral Centre to provide special services in  Eye ,ENT & Dental to children  who are referred from School health clinics .

 

SERVICES  PROVIDED BY SCHOOL HEALTH CLINIC

School health clinic provides comprehensive integrated health care services to school children to achieve the objectives of the school health Program. The services provided to the students include:

·       Routine comprehensive physical health checkup of each student. 

·        Detailed Health Examination of all children to be conducted at 
least at the time of

                                               
          --   Entry to the Middle School                                               
          --   Entry to the Secondary School                                               
          --   Entry to the Senior Secondary School 

The following parameters are assessed in the detailed health examinations:-

 

1.     Nutritional Status by Measurement of Height and Weight

2.      Clinical Examination and Assessment for Anemia

3.      Eye check up for refractive errors

4.      Hearing test

5.      Dental Examination

6.      ENT Examination

7.      General systemic health examination

8.      Assessment of common health disorders
like Intestinal parasites, Scabies, Pediculosis,

9.      Sensory and Motor Examination 

10. Curative services through OPD services.

11. Immunization  

12. First Aid.

13. Referral --
students who require attention of a specialist are referred to referral centers/Delhi govt. Hospitals and subsequent follow up of these cases.

14. Follow- up of referred children

15. Health education (IEC Material). Health Education based on Month and Season of the year. The local and focal diseases are emphasized like in winter- respiratory tract infections, in summer- water borne diseases, in rainy season- vector borne diseases like malaria, conjunctivitis et al.

4. Organization of Health Melas (zone wise) at various schools where on the spot health examination, Eye, ENT, Dental check- ups are done by the specialists. A film on HIV AIDS is screened to create awareness amongst the teenaged students Nukkad natak is performed for spreading the value of healthy diet, anti- Leprosy campaign, dengue fever( its causes and
prevention), effect of  smoking and Tobacco chewing , Tuberculosis, etc. painting competitions, magic shows  are also organized.

FUNCTIONING OF SCHOOL HEALTH CLINIC

The school medical team reports to their respective clinics in the morning and sets up the clinic as per their schedule and the children of respective class are called for routine medical examination. Cumulative health record is maintained for each student. Children needing services of the specialist are referred to specialist with subsequent follow-ups. First aid and Emergency Services are provided as and when required along with OPD services. Incidental health education is given which also covers nutritive value of food and balanced diet for positive health.

 

ACTIVITIES AT CLINIC

School medical Team shifts from main clinic to sub clinics in rotation to provide services to the students of other schools in their respective cluster of schools. Shifting of clinics is preplanned and before shifting information is sent to the Joint Director (I/C) SHS and school authorities. The teams carry all the necessary logistics along with them. In this way all the schools in the cluster are covered in a year. After completing all the schools in the cluster, the teams return to their main clinic.

SPECIAL REFERRAL CENTRE

Referral Centre is opened in each Zone to provide specialized treatment to the students referred from School Health Clinics. Students who require the attention of specialist for the treatment of eye ailment, dental problems & ENT problems are treated in this Special Referral Centers.

CAPACITY BUILDING IN SCHOOL HEALTH SCHEME

For updating the knowledge and skills of Medical officers, Public Health Nurses and paramedical staff, they are regularly deputed to attend various orientation-training programs, workshops and seminars on various subject of national and international importance. All medical officers are trained on various topics like mental health, Basic life support, HIV/AIDS, Refresher course under NRHM etc.

RECENT ACTIVITIES

1.    Diabetes and hypertension screening of school going children as per the recently launched Govt. of NCT of Delhi programme namely Diabetes control programme and Govt. of  India launched programme namely prevalence of diabetes & hypertension in urban slums

            3.  Up gradation of SRCs & SHCs– The presently functioning SRCs have been equipped with RVG machines for performing various dental maneuvers. Various SHCs have also been equipped with AED machine to handle any unforeseen emergencies.

            4.  Fourteen new SHCs have been added to the School Health Scheme since August 2008.

            5.  A separate logo for School Health Scheme has been developed & adopted. This logo is meant for exclusive use by School Health Scheme only.

            6.  Various painting competitions were organized by School Health Clinics to disseminate the Health Education & inculcate Healthy practices & better life style.

            7.  As per the recent directions of the honorable Chief Minister of Govt. of NCT of Delhi we are about to provide first response emergency kit to all the School Health Clinics.

TECHNICAL ADVISORY COMMITTEE (TAG):- A Technical advisory committee of experts from various faculties including sports medicine, social and mental health, orthopedics, Eye, ENT, Dental, adolescent health etc is under process to chalk out various guide lines to fulfill our extended goals:

100 School health Clinics, 11 SRC, 11 Mobile clinics

One clinic for 12 clusters schools. -Total 100 SHCs to be established.

One SRC for each district.-  Total 11Clinics to be established.

One Mobile Clinic for each District -Total 11 to be established.

 

COMPUTERIZATION OF SHS: There are 14 Lac School Children who are covered by SHS. SHS is planning the Computerization of School Health Scheme which would
involve installation of computer and a smart card reader in the clinic. Thereafter all school health clinics will be able to share all the necessary information related to health status of school children. This will also be helpful for monitoring and surveillance. Multiple data related to health status of children will be updated and stored in the clinic computer and smart card issued to the students. There are 36 Parameters to be examined for each student. The total data would have more than five crores data entries. This data will be used for analysis, interpretation and better management of school health scheme. This important data can be shared with any agency or organization at any time which would be of enormous help at the stage of planning various health programmes.

Considering that the data base (five crores approx) to be stored would be enormous computerization will simplify the task.

SMART CARD: SHS is planning to procure smart cards with all the afore mentioned data of the school children which can be handed over to the child when he/she leaves the school. Besides
complete health information, this card will contain personal and academic information including his certificates and other documents in e-form which he can carry in his pocket while travelling.

MASS DEWORM:- SHS is coordinating with an International organization De-worm the world for the de-worming of 14 lakh children of Govt. and aided schools of Delhi.

MASS ORAL HEALTH CAMPAIGN:->SHS is planning to organize an oral health campaign. The objective of this campaign is to promote healthy habits among children & spread a positive health message to the family. 

VIDEO SPOTS & DOCUMENTERY FILMS: - As Audio - visual spots and Documentary Films are very effective promotional tools to create awareness and sensitize the school children , SHS plans to prepare Video Spots & Documentary Films on common health related problems that are prevalent in Indian society .

HOARDING: -School Health Scheme is planning to display hoardings at the gate of each school on healthy eating habits and other health related information.

NEWS LETTER: - SHS is planning to publish a News Letter for the awareness among the staff of educational department and school children on various diseases and health related problems prevalent in school children along with all relevant news and achievements of SHS.

MAGIC SHOW: -SHS provides health education to school children on various topics such as bad habits prevalent in school children, communicable and non communicable diseases & health
related problems in school children by organizing Magic Show in schools. As magic Show is appreciated by school children to a very great extent and they attend to it with great interest the health education message is delivered in a convenient manner thus we are planning to increase the number of magic shows. 

NUKKAD NATAK- SHS is also planning to provide health education to school children on various health topics through Nukkad Natak.

ANNUAL DAY CELEBRATION: -Directorate of Health Services, Govt. of NCT of Delhi started School Health scheme in March 1979, with opening of 6 School Health Clinics. School Health Scheme has completed 31 years in March, 2010. It was unanimously decided by the staff of School Health Scheme in monthly meeting to celebrate the three decades of completion of SHS for critical analysis of the scheme for its betterment.

ROLLING TROPHY: - The SHS is planning to award a rolling trophy to one school health clinic every year on the basis of SHC’s performance. The performance will include medical examination, OPD, TT immunization, Health Education, Punctuality, Innovative ideas, Initiatives for incorporating school health scheme with other Govt. schemes and other extra activities for the
benefit of children undertaken by the clinic.

CO-ORDINATION WITH EDUCATION DEPTT: Since education Dept.
is a beneficiary and in close association, coordination and participation with Education Dept. will lead to a better output. Hence, more meetings with the Education Deptt. are contemplated.

INVOLVEMENT OF NATIONAL AND INTERNAITONAL AGENCIES : – There are some national and international health agencies which are ready to work along with SHS without any additional financial liabilities. These health agencies which share the common health goals are as follows.

Identification of scholar and Sports talents :School health scheme advocates that at the entry point in school the detailed information regarding the interest of student should be identified and then groomed giving more weight age to that talent in addition to routine activities. This will result in overall development of not just the student but also the development of the nation.

HELP LINE:- School health scheme plans to launch a 24 hour help line number to reach out to the children and any other beneficiary

EXPECTED OUTCOME OF SCHOOL HEALTH SCHEME

The school Health Scheme will help School Children by following   manner:-

1.     Promotion of health of the school children through health education and awareness
campaigns.

2.     Prevention of diseases through early detection of diseases and their treatments through
O.P.D. services and referral system.

3.     Substantial reduction in School drop outs.

4.     Increase in school attendance of children.

5.     The healthy children will form the health adults in the population and thereby more
productive population in the country.

6.     A large data would be useful for further implementation and modification of program according to requirement of school Health Scheme.

7.     Disease pattern may easily be identified and measures would be initiated accordingly at
planning level.

8.     This will help to develop better atmosphere amongst Children.

9.     Prevention and treatments of anemia which is very prevalent among school children.

10.A large section of communities will be affected.

11.Students will able to perform better in academically and in sports.

12.As prevention is cheaper and easy to execute thus this scheme will reduce the patient load on hospitals which are already overloaded and thus this scheme may reduce the financial liability of govt. on health sector.
